1. NATURE OF THE MEMBERSHIP
The Membership provides access to bundled clinical services, monitoring, and care coordination as outlined in the tier you select. The Membership is not health insurance and is not a substitute for health insurance coverage. Membership fees are paid directly to the Practice and are not billed to or reimbursed by insurance carriers, though certain underlying medical services may be eligible for reimbursement through your plan.
2. MEDICAL SERVICES INCLUDED
Membership tiers include physician consultations, ordering and review of laboratory work, prescription coordination with licensed compounding and retail pharmacies, dose titration, and direct messaging with the clinical team as defined in your tier. Membership does not automatically include the cost of medications, compounded preparations, supplements, advanced imaging, surgical procedures, or services rendered outside the scope of the Hormone Optimization program.
3. ELIGIBILITY & CLINICAL DISCRETION
Enrollment is conditional on a clinical evaluation and is at the sole discretion of the supervising physician. The Practice reserves the right to decline enrollment, recommend alternative care pathways, or discontinue therapy if it determines, in its medical judgment, that the program is not appropriate for the Member.
4. FEES, BILLING & AUTO-RENEWAL
▪ Membership fees are billed monthly in advance via the payment method on file.
▪ Membership auto-renews each month until canceled in writing per Section 5.
▪ Fees are non-refundable for the current billing month except as required by applicable law.
▪ The Practice may adjust Membership pricing with at least 30 days’ written notice via email and posting on the Practice website.
▪ HSA/FSA eligibility for individual line items is the responsibility of the Member to confirm with their plan administrator.
5. CANCELLATION
Members may cancel at any time by submitting a written cancellation request (email is acceptable) to info@madisonmedicalnj.com. Cancellation is effective at the end of the then-current paid billing month. No long-term contract or cancellation penalty applies. Upon cancellation, ongoing prescription, monitoring, and messaging services through the Membership terminate; existing prescriptions remain valid through their stated expiration but will not be refilled or adjusted under the canceled Membership.
6. LAB WORK & DIAGNOSTIC SERVICES
Standard hormone and metabolic panels are included as defined in each tier. Specialty panels, advanced diagnostics, repeat testing requested outside the standard schedule, and panels ordered for a non-clinical or research purpose may be billed separately at published cash-pay or insurance rates. The Member authorizes the Practice to bill insurance for diagnostic services where coverage exists.
7. PRESCRIPTIONS & COMPOUNDED MEDICATIONS
All hormone preparations are prescribed at the discretion of the supervising physician based on Member’s labs, exam, and symptoms. Compounded preparations are dispensed by licensed 503A or 503B compounding pharmacies in accordance with state and federal regulation. The Practice does not dispense controlled substances directly. Medication costs are paid by the Member directly to the dispensing pharmacy and are not included in Membership fees unless explicitly stated.
8. RISKS, BENEFITS & INFORMED CONSENT
Hormone replacement therapy carries known clinical risks, which are reviewed and documented in a separate Informed Consent at the time of enrollment. The Member acknowledges that no medical therapy guarantees a specific outcome, that individual response varies, and that the Member is responsible for adhering to the prescribed protocol, attending scheduled labs and follow-up visits, and disclosing all medications, supplements, and changes in health status to the clinical team.
9. COMMUNICATIONS & TELEHEALTH
The Practice provides direct messaging and telehealth consultations as part of certain Membership tiers. Communications are conducted through HIPAA-compliant platforms. The Member acknowledges that messaging is intended for non-urgent clinical questions; emergencies should be directed to 911 or the nearest emergency department.
10. PRIVACY & HIPAA
The Practice protects Member health information in accordance with HIPAA and applicable New Jersey law. A separate Notice of Privacy Practices is provided to all patients at the time of enrollment.
11. NO GUARANTEE OF OUTCOMES – MADISON MEDICAL · HORMONE OPTIMIZATION
While the Practice is committed to delivering evidence-based, individualized care, the Member acknowledges that no representation, statement, or marketing material constitutes a guarantee of any specific clinical outcome. Member testimonials reflect individual experiences and should not be construed as representative of all patients.
12. MODIFICATIONS TO THESE TERMS
The Practice may revise these terms with at least 30 days’ notice. Continued enrollment after the effective date of any update constitutes acceptance of the updated terms.
13. GOVERNING LAW & DISPUTE RESOLUTION
These terms are governed by the laws of the State of New Jersey. The parties agree to attempt good-faith resolution of any dispute through direct communication first, and through mediation in Morris County, NJ before initiating any legal action.
